Skilled Home Healthcare

Whether aiding with recovery or managing a chronic illness, Texas Home Health can help individuals achieve their optimal levels of health wherever they call home.


Helping our patients avoid emergency room visits and unplanned re-hospitalizations is at the core of what we do. We help accomplish this by ensuring safe and smooth transitions home from hospital stays and providing professional skilled nursing and therapies, including attention to medication management and fall prevention. Our qualified team members serve patients in Texas with a wide variety of needs with our proprietary RightPath® programs designed for specific conditions including Cardiac Disease, CHF (congestive heart failure), COPD (chronic obstructive pulmonary disease), Diabetes and Late Life Depression. Many of our home healthcare offices carry the distinction of the Home Care Elite award, designating the top 25% of home healthcare agencies among the nearly 10,000 eligible Medicare-certified agencies in the country.

Personal Care Services

Whether as part of a daily or weekly routine, or while recovering after a hospital stay or illness, many seniors find that they could use an extra hand for some of life’s daily tasks. At Texas Home Health, our dedicated care partners and attendants provide a range of personal care services from light housekeeping to bathing and dressing and even companionship, helping to ensure independence at home.

Hospice Care

At Texas Home Health, we offer caring and qualified hands to deliver pain management as well as emotional and spiritual support services, within the familiar comforts of home.


During hospice care, comfort and peace of mind are what matters most for patients and their families. However, hospice care is not “giving up”. Instead, it’s about taking control and making the most of the days to come. Hospice care can allow you to focus on spending quality time with loved ones, while helping to meet care needs and manage discomfort…all in the familiar surroundings of home. Our hospice team members provide a multitude of services from pain management to emotional, spiritual and end of life planning support for patients whose physicians have determined they have less than six months to live, should their disease follow its expected course. We also offer bereavement counseling for families to help them better cope with their loss. Our hospice care offices are accredited by the Community Health Accreditation Partner, CHAP, holding us to the highest standards of care in the nation.6

Through our partnership with the We Honor Veterans program, we offer specialized programs for the unique needs of our patients who have served our country in the military.



A program of the National Hospice and Palliative Care Organization (NHPCO) and the Department of Veterans Affairs (VA), We Honor Veterans provides resources to help extend hospice care uniquely designed for those who have served our country. This program recognizes organizations that demonstrate a consistent commitment to improving care for Veterans with a tiered partnership system. Most of our offices have achieved the highest level of participation, Level Four, helping to ensure the unique needs of our Veterans are met. At Texas Home Health, our employees believe it’s an honor to serve all hospice patients, even when they’re not our own. That’s why AccentCare, our parent company, created the AccentCare Hospice Foundation, which is offered to all Texas Home Health patients.



Supported by personal donations from our employees and by family, friends, or employers of those touched by hospice care, the non-profit foundation offers hospice patients financial assistance to meet needs that are not covered by traditional hospice benefits. Whether providing a warm set of pajamas or covering traveling costs for a loved one’s visit, the Hospice Foundation offers support, peace of mind and often dignity to those who need it most. Hospice care is available for patients whose physicians have determined they have less than six months to live, should their disease follow its expected course. Many individuals wait to enter hospice care until the last days of life, and unfortunately, miss out on many of the benefits hospice care can provide to the patient and their loved ones. Care Management

For qualifying patients in Texas, our care management program will help oversee a network of care providers. This means that patients are able to recover at home with more confidence and less risk of another hospital stay.


Care Management services can include*:

  • Reviewing hospital discharge instructions
  • Assessing home settings for potential accommodations that may be barriers to safety
  • Reconciling medications
  • Identifying community resources
  • Teaching patients about managing their health
  • Arranging for, or attending, follow-up physician visits for assistance in coordination of services and to promote self-care
