For Patients and Families
When it comes to getting well, the old adage rings true: “There is no place like home.” Time and again, patients have shown they do better, both physically and emotionally, in their private and quiet surroundings, close to family and friends.
As you prepare to leave the hospital, appropriate home health care can make all the difference to your quality of life. HRS clinicians and staff members are experienced in dealing with your special home needs, and confident that you and/or a caregiver will be able to learn to administer your care or medication at home. Home care therapy will give you the freedom to recover in the comfort of your own home, and we will help you understand your new responsibilities and answer any questions that you may have.
HOME CARE SERVICES*
- Nursing
- Admitting RN will develop a plan of care around your personal home care needs
- A Patient Admission booklet will be provided to guide you through your care

- RN first visit may take as long as two hours
- RN will educate you and/or caregiver on how to administer your care and/or medication

Telemonitoring technology enables caregivers to monitor and assess a patient’s condition from a remote location. The telemonitor transmits a patient’s vital signs via cell towers from their home to a central computer station where a Registered Nurse monitors the data. HRS works very closely with the patients of hospitals, skilled nursing facilities and health clinics in developing on-site programs for using telemonitoring devices to decrease re-admissions.
Physician Portal
HRS invested in the Allscripts Homecare Physician Portal to streamline communication and to expedite orders for physicians in a timely fashion. The portal is a secure, easy-to-use, web-based application. It is our hope that we can assist physicians with decreasing paper so more time can be spent with the patient.

Data & Outcome Management
Our goal at HRS is to define and provide a higher standard of care through data and outcome management. Every patient, caregiver, employee, and referral source should have the right to review data on how well a service company performs. HRS runs outcome analysis reports weekly and combines them with satisfaction surveys from our patients and employees.
We educate our clinical team regularly on disease state management so that a patient can work toward recovering in the comfort of his or her home. With today’s technology, HRS can review digital trends to determine if a patient or group of patients is getting better through the care they are receiving. Providing a higher standard of care involves understanding patients’ history of care, the transition of care, and ongoing trends of care. Technology, combined with a human touch, will bring a better solution and value to home health care.
Understanding Your Insurance Coverage
Every patient has concerns about medical costs and reimbursement. Home Health Care is substantially more cost-effective than staying in the hospital; however, you will need to understand the HRS uses specific billing and insurance procedures.
An HRS reimbursement coordinator will serve as your advocate in pursuing all the insurance benefits to which you are entitled. HRS will process all of the paperwork for you and will work directly with all public and private insurance providers, including Medicare and Medicaid. Before you leave the hospital, you will be provided a preliminary explanation of what Home Health Care services your individual insurance provider covers. HRS will provide additional deals on specific costs related to your care and update you promptly if your insurance status changes.
QUALITY CONTROL
Consumer Assessment of Healthcare Providers & Systems (CAHPS)
HRS is an extension of the healthcare team that refers a patient. It is our goal to collect information continuously on a patient’s experience so quality can be monitored and improved. All HRS patients are surveyed via telephone on their experiences during an episode of care.
HRS adopts the Home Health Care CAHPS Survey Standard. The CAHPS Survey has been developed by Medicare with the following goals:
- Develop standardized patient surveys that can be used to compare results across sponsors and over time.
- Generate tools and resources that sponsors can use to produce understandable and usable comparative information for both consumers and health care providers.
The various CAHPS surveys ask consumers and patients to report on and evaluate their experiences with HRS. These surveys cover topics that are important to consumers and focus on aspects of quality that consumers are best qualified to evaluate, such as the communication skills of providers and ease of access to health care services.
All CAHPS surveys are in the public domain, which means that anyone can download and use these surveys to assess experiences with HRS. Users of CAHPS survey results include patients and consumers, quality monitors and regulators, provider organizations, health plans, community collaborators, and public and private purchasers of health care. These individuals and organizations use the survey results to improve decision-making and the quality of health care services.
For Healthcare Providers
Digital Care Pathways
HRS meets with hospitals regularly to define patient needs after discharge. HRS then assesses the patient in the home environment. Socio-economic needs and clinical needs are assessed in the home environment to determine a living care pathway that the HRS clinician can follow. By standardizing a route for our clinicians, HRS is able to define and refine better methods of care. HRS is continuously developing care pathways related to specific disease states. The care pathways are uploaded within our software system so clinicians can access them on individual tablets.
Telemonitoring
What is a Home Health Monitoring System?
Imagine the sense of security and peace of mind for patients and caregivers who know their health is being monitored by trained, medical personnel on a daily basis. HRS Home Health patients can have a sense of security using state-of-the-art telehealth technology. Telemonitoring technology enables caregivers to monitor and assess a patient’s condition from a remote location. The telemonitor transmits a patient’s vital signs via cell towers from their home to a central computer station where a Registered Nurse monitors the data. Using voice prompts and touch screen capabilities, the telemonitor can record:
- Weight
- Blood Pressure
- Oxygen Saturation
- Pulse
All of the above data is immediately transmitted to the HRS Corporate Office for clinical review. If the data collected indicates a need for intervention, or if vital signs are not collected when prompted, appropriate action is taken by members of the HRS care team.
