What you should know…
At AccentCare, data isn’t just numbers on a screen — it’s a powerful tool that helps ensure our patients receive the right care at the right time. By combining compassion with clinical expertise and backing it all with meaningful insights into your health condition, we deliver care that’s both personal and precise.
Data-driven personalization
Data plays a transformative role in tailoring treatment plans to a patient’s unique needs. It allows your care team to anticipate complications, adjust interventions proactively and optimize treatments in real time. Every data point used is specific to your care journey, helping clinicians make informed decisions that align with nationally recognized clinical guidelines and your personal health goals.
Evidence-based protocols
Evidence-based care is central to AccentCare’s RightPath® program (opens in new tab) offerings — a structured approach to home health that blends clinical expertise with personalized support. These protocols are grounded in national guidelines from leading organizations like the American Heart Association, American Diabetes Association and the National Consensus Project for Quality Palliative Care.
RightPath care plans are built using a triad of inputs: national guidelines, physician instructions and your personal goals. This ensures your care is clinically sound and relevant to your lifestyle, recovery objectives and health challenges.
The results speak for themselves. Patients enrolled in one of our RightPath programs — such as COPD, Cardiac Care and Joint Rehabilitation — experience fewer 30-day hospitalizations than the national average.
Predictive modeling
To equip teams with the information to respond proactively with increased support and tailored interventions, AccentCare has introduced Medalogix Muse. With 90% accuracy, this predictive tool analyzes real-time electronic medical record (EMR) data to identify when a patient may be within seven days of expected death.
When a patient is flagged as high-risk, AccentCare increases visits from registered nurses and social workers. This allows time for additional pain management, emotional support and comfort care, while reducing avoidable emergency room visits and hospitalizations. The result is a smoother, more dignified end-of-life journey for patients and better-informed families.
While clinicians will always make patient care decisions and provide direct care, tools like Muse enable better clinical results for patients and families. Predictive modeling doesn’t replace human compassion — it enhances it.
Outcome analytics
AccentCare continuously monitors and adjusts care based on clinical outcomes and patient experience. Tools like OASIS and CAHPS® surveys provide valuable insights:
- Outcome Assessment Information Set (OASIS) captures data on health conditions, treatment needs and risk factors throughout care. Clinicians input this data, which is used by the Centers for Medicare and Medicaid Services (CMS) to guide quality improvement and calculate provider performance.
- Consumer Assessment of Healthcare Providers and Systems (CAHPS) surveys measure patient experience and help increase transparency through public reporting, allowing meaningful comparisons between health care providers. CAHPS surveys (opens in new tab) are administered by CMS and distributed to patients, inquiring about their experiences with health care providers and plans, like hospitals, home health agencies, drugs plans and more. Patients and family members can access this data publicly through the Medicare Care Compare (opens in new tab) website.
- Hospice Outcomes and Patient Evaluation (HOPE) is a hospice quality reporting tool, capturing patient and family needs in real time, informing quality measurement and improvement.
