AccentCare Hospice is there every step of the way. We work with a wide interdisciplinary team to create plans of care that address clinical needs and pain management, while allowing patients and families to play an integral role in their care decisions. Hospice care incorporates personal wishes and family support, including end of life planning and counseling.

Our specially-trained interdisciplinary hospice team includes:

Attending Physician:

• Certifies terminal diagnosis
• Manages symptoms, prescribes appropriate treatment and approves the plan of care
• Coordinates with interdisciplinary team and participates in the plan of care.

Medical Director:

• Certifies terminal diagnosis
• Oversees pain and symptom management for patient comfort, as needed or requested
• Participates in the development of the plan of care

Nurse Case Manager:

• Assesses patient and family needs
• Manages pain and symptoms, as well as educates the patient and family on disease progression
• Participates in the development of the plan of care to help ensure patient needs are met

Hospice Aide:

• Provides direct personal care for the patient
• Applies comfort measures and reports identified needs to the Nurse Case Manager
• Offers emotional support for patient and family

Bereavement Coordinator:

• Provides pre-bereavement assessment and counseling
• Offers bereavement support groups and referrals to community services
• Delivers individual counseling

Spiritual Care Coordinator:

• Assesses patient and family spiritual needs
• Facilitates life review discussion
• Provides consultation to clergy

Social Worker:

• Assesses patient and family social, emotional and financial needs
• Assists with advance care planning
• Provides direct counseling or refers patients and families to appropriate community agencies

Volunteers:

• Offer companionship and support to patient and family
• Deliver needed non-medical services and respite time for family
• Provide support at time of death and during bereavement
