Who qualifies for home health?

Your patient is eligible if they meet all of the following criteria:

• Under the care of a physician
• Homebound (require considerable effort or assistance to leave home)
• Face-to-face encounter with a physician or advanced practice provider (APP) within 90 days before or 30 days after start of care
• Need for intermittent skilled care: nursing, physical therapy, speech therapy or occupational therapy

Skilled needs

• New diagnosis
• Uncontrolled diagnosis (e.g., unstable CHF, COPD, diabetes, anxiety, depression or dementia)
• Recent stroke, fall or surgery (opens in new tab)
• Change in medication
• New treatment (catheter, glucose meter, nebulizer, ostomies)
• Wound care or infection management
• Gait, strength or balance issues (opens in new tab)
• Stroke rehabilitation (opens in new tab)
• Behavioral health concerns (opens in new tab)

To deliver timely, effective care, we rely on complete and accurate orders. Incomplete referrals delay treatment and can compromise patient safety.
