AccentCare Southeastern Health Care at Home cardiopulmonary care includes Cardio Care and cardiac rehabilitation. AccentCare Southeastern Health Care at Home physician-led clinical services work with patients, caregivers and providers to develop a plan of care that can be delivered in the home while maintaining the level of care necessary to ensure that patients are successful in their treatment.
The Cardio Care Program is a home-based Heart Failure Disease Management program that is focused on:
- medication management
- physician follow-up
Home-based cardiopulmonary rehabilitation combines clinical care, daily exercise programs and treatment, and Remote Patient Monitoring. Cardiac rehabilitation services focus on one or more of the following:
- Heart Failure
- Chronic Obstructive Pulmonary Disease (COPD)
- Transitional Care after surgery or cardiac event
All cardiopulmonary home health services are delivered by AccentCare Southeastern Health Care at Home professionals who ensure the clinical integrity of the services, while providing the care and compassion that make a difference to patients and caregivers.
All therapists deliver education and home monitoring with care and compassion. They coordinate with patients’ physicians to design daily exercise programs and treatments to improve activity levels, safety, and overall quality of life.
Heart Failure Disease Management
AccentCare Southeastern Health Care at Home CardioCare Program provides assessment and education to empower patients to manage their own chronic diseases.
Management emphasizes three core principles:
- Medication management
- Physician follow-up
The program follows an evidence-based, clinical protocol that is designed by physicians to avoid emergent transfers to the hospital and improve quality of life. The program includes patient and family education for healthy lifestyle choices and self-monitoring, and integrates modern technology, including Remote Patient Monitoring.
High-tech infusion therapy by skilled clinicians is available when needed. Milrinone (cardiac inotropes) and IV Lasix can be provided in the home.
AccentCare Southeastern Health Care at Home delivers home-based cardiopulmonary rehabilitation, allowing patients to receive clinically specialized care in the place they feel most comfortable—their own homes.
Heart Failure and other Cardiac Rehabilitation services include:
- Education to prevent hospitalization
- Treatments to increase exercise capacity, improve circulation and help the body use oxygen more efficiently to increase activity tolerance
COPD and other lung-related rehabilitation includes:
- Breathing techniques and exercise
- Energy conservation training
- Posture correction to improve breathing
- Treatments to increase endurance with activities of daily living
- Spirometer for Remote Patient Monitoring
Transitional care after surgery or cardiac event includes:
- Education and training in performing daily tasks and functional mobility within sternal or cardiac precautions
- Designing daily exercise and walking program
- Using Remote Patient Monitoring to monitor vital signs and response to activity
