That’s why AccentCare Southeastern Health Care at Home New Directions primary care provides all of these things and more.

Trust when you choose New Direction primary care, you have chosen excellence. As the population of US citizens over the age of 65 increases, illnesses and functional limitations often make it difficult and taxing to get to planned medical appointments. This often results in missed appointments, which can lead to unwanted emergency room visits or hospital stays. New Directions is a home-based service designed to improve access to medical care for individuals who might otherwise have difficulty getting to the doctor. These personalized home-based visits allow the provider to spend more time on your unique needs and they bring comprehensive care right to your home.

What is New Directions Primary Care?

New Directions is a proud provider of home-based primary care, which is intended to meet the specific needs of patients who have difficulty accessing medical care. Our team of Certified Registered Nurse Practitioners (CRNP’s) work collaboratively with their physician advisors to optimize care and ensure their patients’ quality of life.


New Direction’s Mission and Care Model

The mission of New Directions primary care is to assist patients in avoiding unplanned trips to the hospital and to improve the quality of life of those individuals with complex medical needs.

Dr. Jennifer Minkovich, DO

Dr. Jennifer Minkovich is a native Philadelphian and attended all levels of her schooling in the Philadelphia area. She earned her undergraduate degree from Temple University.

She then attended Philadelphia College of Osteopathic Medicine, where she first earned a graduate degree in Biomedical Science and then went on to complete her medical degree. She completed her Family Medicine internship at St. Joseph’s Hospital and her Family Medicine residency at Aria-Jefferson Hospital.

Her areas of interest include Women’s Health, Palliative Care and Medical Cannabis/Cannabinoid Science.

Dr. Minkovich views the physician-patient relationship as a partnership and finds joy in advocating for her patients and providing them with guidance and encouragement in their individual paths to health and wellness.

Dr. Minkovich is Board-Certified in Family Medicine and Osteopathic Manipulative Medicine. She currently lives in the Bucks County area with her husband and four fur-babies. She loves hiking, gardening, and kayaking.


Betsy McLaughlin, CRNP

Betsy McLaughlin is a Board-Certified Adult-Gerontology Nurse Practitioner. She received her Bachelor of Science in Nursing from Holy Family University and Nurse Practitioner Master’s degree from Gwynedd Mercy University in Adult and Gerontology Primary Care.

She went on to receive her certificate in Advanced Practice Palliative Care from the California State University (CSU). She has over 30 years of nursing experience in various settings, including several administrative positions for Southeastern Health Care at Home.

Betsy’s entrepreneurial spirit and passion for home-based care was the impetus for New Directions Primary Care’s beginnings. Her patient care approach is “patient-centric.” She believes the best approach to wellness is to empower patients to be an active participant in their healthcare.

Betsy enjoys spending time with her husband, children and her fur babies.


Cara Sottile, CRNP

Cara attended La Salle University where she obtained her Bachelor of Science in Nursing in 2010. She went on to obtain her Master’s Degree at Drexel University graduating Magna Cum Laude in 2017 as an Adult-Geriatric Primary Care Nurse Practitioner.

Cara is a proud member of Sigma Theta Tau-international Honor Society of Nursing and she is certified with the American Nurses Credentialing Center as a nurse practitioner. Cara has over 10 years of critical care experience. She enjoys sharing her knowledge of nursing and served as an adjunct faculty member/clinical instructor at the University of Pennsylvania.

Cara’s spare time is spent volunteering in Haiti. She is co-founder and chairwoman for One with Earth (OWE), providing food and education to children in orphanages.

Cara feels strongly about making a positive impact in the lives of adult-geriatric patients. She loves to travel having traveled to 23 different countries and over 30 U.S. states.


Dr. Katherine Stackhouse DNP, FNP-C, ACHPN

Dr. Katherine Stackhouse is an Advanced Practice Registered Nurse (APRN) serving in a home based primary care practice with New Directions.

She is board certified as a family nurse practitioner combined with further post graduate studies and board certification in hospice and palliative medicine. For more than 13 years she has served in a wide variety of primary care roles.

Dr. Stackhouse possesses an enthusiasm to share nursing knowledge and evidence-based practice, fostering the future of nursing practice. She teaches in the graduate nursing program at DeSales University in Center Valley, PA.

She maintains an active clinical practice as a nurse practitioner for patients and families surrounding her local community. Additionally, she provides education, mentoring, and orientation for Palliative Care APRNs for a large, nationwide hospice and palliative care program. Lastly, serves as an educational resource content experet and volunteer for the Hospice and Palliative Nurses Association.

Dr. Stackhouse is a member of the American Academy of Nurse Practitioners, American Association of Nurse Practitioners, National Organization of Nurse Practitioner Faculties, Pennsylvania Coalition of Nurse Practitioners, and Hospice and Palliative Nurses Association.

She has a great love for nature and travel, exploring new places and cultures. Whether it be the beach or the mountains, she enjoys everything nature has to offer.

Dr. Stackhouse received her BSN from Cedar Crest College and her MSN/FNP from DeSales University. She completed her DNP at DeSales University with a concentration in Clinical Leadership.


Jaymee Weitzman, CRNP

Jaymee Weitzman is a Board-Certified Adult-Gerontology Nurse Practitioner. She holds a Master’s Degree in Nursing from LaSalle University in Philadelphia and, prior to this, a Bachelor’s degree from Drexel University.

Before attending graduate school, she worked at Einstein Medical Center, then went into long-term care, working with the adult and geriatric population. This is where she saw her passion for nursing grow, becoming the Director of Nursing role before furthering her education and becoming a Nurse Practitioner. She also serves on the advisory board for the Bucks County Community College nursing program.

As a Nurse Practitioner, she has worked with the aging population specializing in long-term, primary, post-acute, and home care. She is dedicated to providing patients and their families with individualized, compassionate care.
In her spare time, she enjoys spending time with her 2 young children and her husband as well as traveling.

Routine, medically necessary visits are made Monday through Friday from 8:30 AM to 5:00 PM. We are available by phone 24 hours a day, 7 days a week for after-hours sick calls and referrals.
