The AccentCare Southeastern Health Care at Home Wound Care Program focuses on the treatment of wounds and improving outcomes, as well as the patient’s overall health. The program is led by a Certified Wound Ostomy Continence Nurse (WOCN) with the assistance of a team of specially-trained “Wound Champions.”

AccentCare Southeastern Health Care at Home provides one of the only WOCN home-based programs in the region, providing highly-skilled nurses to care for vulnerable patients. The contact AccentCare Southeastern Health Care at Home team achieves optimal healing at home by following the evidence-based treatment protocols based on standards of care established by the WOCN Society. This includes continuous staff training and education and the development of individualized treatment plans. Other components of the WOCN Society protocol include:

    • Optimal nutrition for healing
  • Weekly case conferences to discuss best practices for healing wounds and decreasing healing time

  • Collaboration with physicians wound care centers, and other multidisciplinary team members
